Tell us about your family before your appointment.
After booking, you’ll receive an intake form by email to complete before your first visit. We will reach out to you to confirm your appointment.

Insurance Coverage by Plan
Anthem Blue Cross Blue Shield Coverage
Gwinnett Breastfeeding now accepts select Anthem Blue Cross Blue Shield plans for virtual lactation consultations through the Wildflower Health partnership.
Eligible Anthem members may receive:
• Up to 2 prenatal lactation consultations
• Up to 6 postpartum lactation consultations
• 100% covered virtual visits for qualifying plans*
Because Anthem requires enrollment through its Building Healthy Families program, there is a simple 2-step process before scheduling.
Step 1

Complete Building Healthy Families Enrollment
After approval, Anthem members must finish enrollment in Anthem’s Building Healthy Families program.
You are fully enrolled once you reach the “Welcome” screen.
Important:
Do NOT click any buttons labeled “Lactation” inside the Anthem portal because it will not route you back to Gwinnett Breastfeeding.
What Visits Are Covered?
Eligible Anthem members may receive:
• Up to 2 prenatal lactation consultations
• Up to 6 postpartum virtual lactation consultations
• Fully covered virtual support for qualifying plans
This includes support for:
• painful latch
• low milk supply
• pumping support
• return-to-work planning
• bottle refusal
• feeding transitions
• breastfeeding questions and education
Aetna
We’re in-network with most Aetna plans. You can use your benefits for in our office care or in your home.

COVERAGE
- Mom and baby will be covered under the plan if both are insured by Aetna.
- If only the mother is on the plan, we will bill for mother.
- Mother must be pregnant, breastfeeding, or pumping to use Aetna benefits.
- We do charge a travel fee for home visits.
Cigna
We accept most Cigna PPO plans, and Cigna Marketplace plans. To be instantly approved for coverage, please use this link with Wildflower. After booking, you’ll receive an intake form via email to complete prior to your first visit.

Home Visit: (90 minutes or 75 minutes): $150 + travel fee
United Healthcare
We’re in-network with most United/UMR plans and happy to help you use your benefits for in-home or in-office care. Most United/UMR plans cover lactation care with no cost-sharing.

COVERAGE
- Mom must be pregnant, breastfeeding, or pumping to use their insurance
- Prenatal consultations and group classes are billed for mother only.
- We will bill both mom and baby at each lactation visit.
- You can use your insurance for our Pumping To Win consultation.
Tricare
We are in-network with Tricare-East and accept this plan. Your plan will pay for 6 visits. If you are pregnant, Tricare will pay for a prenatal lactation consultation. Mother must be breastfeeding, or pumping to use Tricare-East plan. We do not do home visits for Tricare-East.

- Tricare will pay for a prenatal lactation classes
- Mother must be breastfeeding, or pumping to use Tricare-East plan
- We do not do home visit for Tricare-East
Medicare, Medicaid, and most HMO
We are not in network with Medicare or Medicaid. Patients with these plans will pay out of pocket. We do offer a lower rate if you want to still receive breastfeeding help. Mom and baby will be covered with this payment. Mother must be pregnant, breastfeeding, or pumping.

We are in-network with Christian Ministry and accept this plan.

We are in-network with Ambetter and gladly accept this plan.

Home Visits: $300 for new patients and $275 for established patients

Not every insurance plan covers lactation care, but that should never prevent you from receiving expert breastfeeding support.

Blue Cross Blue Shield Members
While Gwinnett Breastfeeding is not currently in network with Blue Cross Blue Shield, many families receive significant reimbursement for lactation consultations after submitting their superbill to BCBS. If you have questions about reimbursement, we're happy to help guide you through the process.
